How do I begin?
Call us at 802-888-4651 and ask for a Long Term Care Specialist. She will help you determine which of our programs might fill your needs. The options include Medicaid funded programs such as Choices for Care, Traumatic Brain Injury (TBI), Pediatric and Adult High Technology. Our Care+ Program is a full-fee service.

Who might come to my home?
A nurse, personal care attendant, case manager or home health aide depending on the program.

Who pays for this?
Medicaid and occasionally, Long Term Care insurers, private pay.
