Our Telehealth Program monitors a client's vital signs and symptoms via a machine that transmits the information to Lamoille Home Health & Hospice. Telehealth monitoring has been shown to improve the delivery of home care and to decrease unplanned emergency visits and hospitalizations. 


Wound, IV and Ostomy Care

This treatment maximizes the wound healing process for:

  • Burns

  • Diabetic foot ulcers

  • Pressure ulcers

  • Surgical wounds

  • Radiation wounds

  • Skin tears

  • Ulcers due to poor circulation

  • Venous ulcers


Ostomy Care

Ostomy care consists of:


  • Ostomy self-care education

  • Peristomal skin evaluation and care

  • Stomal care including problem solving for stomal equipment and fitting, skin irritation, rash, leakage, odor and ulcerations.

Better Breathing Program

Our Better Breathing program is for people with diseases that affect their breathing and impairs their ability to perform daily activities. These include Chronic Obstructive Pulmonary Disorder (COPD), pulmonary fibrosis, lung and congestive heart failure, serious asthma and neuromuscular disease. You must also be a non-smoker or working toward smoking cessation and have an order from your doctor.

The Better Breathing program helps you regain your quality of life using specialized therapeutic approaches including:


  • Exercise training and breathing strategies.

  • Energy conserving techniques.

  • Postural training.

  • Anxiety coping strategies.

  • Education about their disease.

  • Medication management & instruction in the use of medication delivery device


Falls Prevention

Our program provides techniques for elder and frail adults to help reduce their fear of falling, teaches exercises that can strengthen muscles, improve balance, and foster awareness of other techniques and tips that will reduce falls. For more information about our Falls Prevention program, contact the LHH&H Physical Therapy Department at (802) 888-4651.

Chronic Disease Management

Lamoille Home Health & Hospice can help manage the impact of your condition, help you to regain your independence and achieve the best possible health and quality of life. 

Our care services for persons with an illness include: 

  • Assessment of physical status and vital signs

  • Care coordination between LHH&H, your physician and other community services

  • Education about your disease

  • Assistance with managing your medications

  • Support in activities of daily living such as bathing and dressing

  • Social worker support, including advance directive planning and counseling for you and your family.



Lactation Consultation and Support

New mothers adjusting to life with a newborn can be challenging on many levels.  LHH&H provides breastfeeding information and support to mothers. We provide support, training and guidance to breastfeeding mothers and their babies, helping mothers achieve their nursing goals. We can also provide pumps and equipment that some mothers need to make breastfeeding work for them.
